CAN Services

CAN Community Health has been serving the needs of the HIV community in Florida and now South Carolina for over 26 years. We are a private, not-for-profit organization dedicated to the treatment, care and continual wellness of people living with HIV.  Our clinics offer the finest medical, dental, psychological care and lifestyle counseling to all patients, regardless of their financial situation, insurance status or ability to pay.

Comprehensive Care

CAN practices comprehensive care involving the expertise of many different healthcare professionals to manage the multiple physical and emotional health problems of a patient. CAN believes in treating the whole patient and providing services specific to the needs of the patient.

An integral part of our service is to remove obstacles to care through case management, support programs, such as Ryan White, housing, etc.

 

Medical Care

Attending physicians provide and coordinate clinic-based medical care with nurses and other specialized health care providers.

Dental Care

Many CAN locations offer dental services for patients living with HIV including:

  • Oral hygiene education
  • Preventative oral care
  • Emergency care
  • Restorative dental care

Counseling and Therapy

Mental health counselors provide therapeutic counseling to couples, groups and individuals to complement medical services in treating the whole patient.

Case Management

  • Case managers determine patient eligibility for state and federal programs
  • Develop individualized plans of care and monitor progress toward patient-centered goals
  • Coordinate patient referrals to community services
  • Insurance liaison
  • Housing assistance

Education & Peer Support

Effective HIV education can help prevent new diagnoses by providing people with information about HIV and how transmission occurs. In doing so, we equip individuals with the knowledge to protect themselves.

Our Education Program brings the latest information about HIV to many schools. We also hold events to reach out to those in our community who are particularly at-risk, including teens and minority populations. These include Community Awareness activities and Event and  additional Patient Education.

Pharmacy Services-Coming Soon!

CAN Community Health will open several new in-house pharmacies soon to make it easier for patients to receive the best possible care. Many patients have to arrange for transportation to appointments and by offering them the ability to get prescriptions while at their appointment saves time. Having a dedicated pharmacy also provides greater privacy and security to patients.
