    INSURANCE: CAN Community Health accepts a wide variety of insurance across all of our locations.  Please contact each location directly for a participating plan.


    FINANCIAL ASSISTANCE: CAN Community Health offers financial counseling and assistance programs to all patients who qualify.  Sliding Fee Discount Programs are offered to all uninsured patients below the 400% Federal Poverty Level.  At least one month’s Proof of Income is needed to qualify.  Please reach out to our Financial Counselor at 844-922-2777, Ext 11975 for more information.  You may also bring this information to your visit and our Patient Experience Reps/check-in staff will help you apply for this program.

    STATEMENTS: Paper statements will be mailed once per month. Please make sure your address stays current. Patients with a Patient Portal will receive an electronic statement in your portal account and a paper statement. Patients who prefer to receive an electronic statement only should call 844-922-2777, Ext 11970 to request to Opt out of paper statements.

    Please note: If you transfer your services out of CAN Community Health, you will automatically receive a paper statement for outstanding balances.

    ONLINE BILL PAY:  As a web-enabled/portal user, you have access to your account 24 hours a day.

    NO SURPRISE ACT:  Can Community Health values patient choice in healthcare and financial transparency.  The No Surprise Act protects you from surprise billing and requires us to provide you with a better understanding of your out-of-pocket costs.  As an uninsured patient, Good Faith Estimates will be provided to you prior to your visit.  Patients interested in requesting an estimate for services may contact Customer Service at 844-922-2777, Ext. 11910 Monday – Friday, 8:00AM – 5:00PM EST.

    IMPORTANT MESSAGE:

    All clinics in Florida, with the exception of Pensacola and Fort Walton Beach, will be offering Telehealth services on Monday, 10/7.
    All CAN Pharmacies will stay open until 5:30 PM on Monday, 10/7, and will be available by phone for any urgent matters during closures.
    All Florida clinics, (except Pensacola and Fort Walton Beach) will be closed on Tuesday, 10/8, Wednesday, 10/9, Thursday, 10/10, Friday 10/11, and Saturday 10/12.
