Eligibility Requirements
All persons interested in HIV/AIDS Patient Care Programs must complete an application and bring in the required paperwork. The following are the basic eligibility requirements:
- HIV positive status
- Living in Florida
- Low income (currently at or below 400% of Federal Poverty Level)
All applicants will provide written proof that they meet the above eligibility requirements as determined by the Florida Department of Health. Proof includes the following:
- Proof of HIV+ status (ask a Case Manager about specific requirements). A doctor’s statement does not satisfy Florida Department of Health requirements.
- Proof of living in Florida (current photo ID and other proof of living in Florida, i.e., utility bill, bank statement, pay stub, current lease, etc.)
- Proof of receiving services from other programs or that you have been denied services (Medicaid/Medicare cards, health insurance card, public assistance award or denial letters, Veterans Administration documents, etc.)
- Proof of income (3 months of pay stubs or pay stubs showing year-to-date gross income, 3 months of bank statements, statement from Social Security called PEBES, rental or other income, self-employment income documentation, public assistance award/denial letters, current unemployment compensation statement, support letter from person providing financial assistance, etc.). Call a Case Manager to get more specific requirements.
Applicants must be willing to provide all the required paperwork and sign the forms. If an applicant is unwilling to satisfy documentation requirements or does not meet the eligibility criteria, then a letter explaining why the applicant is not eligible will be provided. The decision may be appealed and information on the appeal process will be given.
